It depends on what you are taking it for. Iron-related markers changed fastest in the research, with several pregnancy trials reporting differences within 30 days. Most other outcomes, including inflammation, immune and skin measures, were assessed at 8 to 12 weeks. The median trial duration across the published literature is 12 weeks.

Timeframes reported in the research
| Outcome studied | When it was measured |
| Iron markers (haemoglobin, ferritin) | Changes reported within 30 days in pregnancy trials |
| Visceral fat area | 8 weeks, at 300 mg/day enteric-coated |
| Inflammatory markers and immune measures | 4 to 12 weeks, median 12 |
| Skin lesion counts and sebum | 12 weeks, assessed monthly |
| Gut microbiome composition | 28 days |
Why it is not faster
Lactoferrin works through gradual processes rather than acute ones: inflammatory signalling, microbial balance in the gut, and the hormonal regulation of iron. None of those shift overnight. Studies running shorter than about four weeks rarely report much of anything, which is a reason to be sceptical of any product promising results in days.
What this means practically
Give it a consistent 8 to 12 weeks before drawing conclusions, taken daily. The trials that reported effects were trials of daily use over that kind of period, not occasional use. If your reason is iron-related, you may notice changes sooner, though only a blood test will tell you what is actually happening to your iron markers.
